If you’re here you’ve obviously considered giving one of these wonderful dogs a home! We’d love to help you find the perfect dog, you might have one in mind already. There are so many lovely dogs out there waiting for a chance at their forever home, and we want to make sure they find the perfect family as much as we want you to find the perfect dog.

So whilst this form may look a bit long, we ask that you take your time going through it and filling it out as thoroughly as you can. We can’t wait to help you find the newest addition to your family!

A few things before we get started

  • Before completing this form, please ensure you have read and understood the Terms and Conditions included in this adoption pack.
  • Adopting a dog is a long-term commitment (10 – 18 years in the case of a puppy), please ensure you can take on the care of a dog, both practically and financially, before deciding to submit your application.
  • Please complete all parts of this form and include as much info as possible, including photos of your garden and areas of the house where the dog will be allowed (there is a place to upload them in the form).
  • We reserve the right to refuse any adoption that we feel will not suit the dog/puppy’s requirements.

    I understand that I must keep Starlight Barking informed of any change in cirumstance affecting my dog – including change of address – and if, at any time, I can no longer keep my dog, I must contact Starlight Barking and make arrangements to return the dog to a volunteer.

    I certify that I am at least 21 years of age and the information I have given is true. I recognise that any misrepresentation of facts may result in my losing the privilege of adopting a companion animal, and I understand that Starlight Barking has the right to deny my application.

    I am fully aware that I am adopting a living creature and as such that Starlight Barking is unable to guarantee the health of the animal. If this animal becomes ill, I certify that I am financially and emotionally prepared to treat this animal at my own expense.

    • All adoption questionnaires are reviewed with the sole intent of making the best long-term companion match possible. You will be informed as soon as possible as to whether we feel you are a match for the dog.

    What will it cost?

    Donations:

    We appreciate donations of any amount as we recognise that everyone has different financial circumstances. The adoption donation contributes to the cost of transporting the dogs from the island to the UK - their health check, vaccines, passport and micro-chip and onward travel to their UK home. As the travel costs per dog are now in excess of £1,000 we do please request a minimum donation of £650 for adult dogs and £750 for a puppy under a year old. We are incredibly grateful for any amount above this to help rescue a dog from an impoverished and neglected life. The rescue dog will remain the property of Starlight Barking until the donation has been made.

    No dog will be reserved until completed form and donation payment have been received.

    Adoption Terms and Conditions

    Homecheck: We endeavor to have home check carried out via facetime as soon as possible after submission of this form – ideally within 2 weeks.

    Neuter/Spay: Most of our rescue dogs are neutered before flying, but on the occasion that they are too young for surgery, it is a compulsory condition of your adoption that you commit to neuter or spay your puppy once they are old enough and provide a vet’s written confirmation that this has been undertaken. This will be part of our follow up checks. Puppies under the age of 8 months and subject to development, may not be neutered/spayed before travel, and we will advise on an individual basis. Starlight Barking will contribute £65 to the neutering of a dog on production of the vet receipt/invoice.

    Return of dog: Starlight Barking MUST be informed at any time of a change in circumstances and the dog returned to one of our volunteers where required. Our dog must never be handed over to any UK Rescue. We always accept the return of our dogs and will endeavor to rehome them accordingly.

    Age/Breed: Starlight Barking CANNOT guarantee either the age, or the breed (or breeds), of a dog.

    Follow Up: Starlight Barking will stay in touch with you, and retain the right to also carry out a further visit to your home after the dog is placed, as a follow up check. Starlight Barking have the right to remove the dog from your care if your circumstances have changed to the degree that they no longer meet our rehoming criteria and, as such, would fail any future follow up home checks.

    Health: Starlight Barking recommend that the dog is registered with and visits a vet within 5 days of being homed for a health checkup.

    All dogs are treated for parasitic infection before departure from Greece, but we recommend you take advice from your vet regarding treatment on arrival, as we cannot guarantee that the treatment administered upon departure will completely eradicate any parasitic infection.

    All our Dogs are blood tested for Heartworm, Ehrlichia and Leishmania (a disease common in Mediterranean countries) prior to travel. It is possible that dogs which return a negative Leishmania result before travel may already infected. It can remain dormant for several years with no clinical signs or symptoms. Stress factors can cause the Leishmania to become active. Dogs can be treated effectively & live a happy life, with daily medication. It is wise to blood-test the dogs at least once a year for a couple of years. The test should be done sooner, of course, if any symptoms of Leishmaniasis are noted. Cases are extremely rare, but they can occur years after travel. Please ensure you have your dog fully covered by pet insurance

    Pet Insurance: Please ensure that you have a full Pet Insurance in place. Should you choose not to take out pet insurance, it is a condition of adoption, that any future medical needs will not be restricted in anyway due to personal finances.

    Microchip: All our dogs are microchipped prior to travel, and are registered under our charity with Petlog, for their safety at the point of entry to the UK. It is a legal requirement that you update the registration with your own details, and keep it up-to-date, this can be done online at www.petlog.co.uk

    Settling in: We want your dog to settle in to your home, but some do take longer than others, so please be patient. We do not want dogs returned within a few days of adoption/foster. You will not see the true character for possibly weeks to several months. Your dog needs time to get to know his new family and his new home and recover from his transition.

    Refund Policy: We offer a refund policy in the unlikely event that you need to return the dog, we will rebate up to £400 of your adoption donation during the first 2 months and £300 after 2-4 months (subject to the dog being returned to one of our foster homes once space is available and being adopted again).

    Starlight Barking reserves the right to refuse any application they deem unsuitable.

    DATA PROTECTION/PRIVACY

    1. Your personal data will only be used in connection with your application/adoption. Starlight Barking will never pass your information to any third party not connected to the process, and the details will not be used for marketing purposes of any third party. We may contact your vet to confirm neutering/vaccination/other medical details either about your current dog(s) or your adopted dog, both before and after adoption.
    2. If you are successful in adopting a dog your details will be held on file for the lifetime of the dog. We provide lifetime rescue back up for each dog, and by completing our Adoption Contract, you agree to this.
    3. Third parties, will include the Pet Courier, foster homes i.e. Central Bark, sharing between the UK Charity and Greek Rescue to establish the Petlog and Vet Services registration
    4. All returned applications are only held within our Charity via our IT Services and emails and any hard copies within our offices, in a secured environment

    The Adopter agrees to comply with all relevant regulations, legislation and laws concerning the welfare, control, responsible ownership and protection of animals including without limitation the Animal Welfare Act 2006 and Codes of Practice issued in England and Wales under the 2006 Act and to ensure that anyone with the temporary care of the Animal also complies. This means providing for the animal’s health and wellbeing, and taking measures to ensure that the animal’s health is maintained, he/she is properly housed, fed, watered, socialised and exercised and receives appropriate veterinary care, including vaccination, regular worming and flea treatment.

    By submitting this form you have read and understood the above terms and conditions and agree to our conditions if you proceed with an adoption. Once we approve your application and confirm it to you, then is deemed as both parties have fully accepted the terms and conditions under England and Wales Law.
